Maxillofacial Surgery


The Complex Operative Unit of Maxillofacial Surgery and Otolaryngology (ENT), located on the 6th floor, Staircase E, provides inpatient care and treatment for patients with conditions affecting the cervicofacial-maxillofacial region.

The Unit integrates Maxillofacial Surgery and Otolaryngology, creating a unique structure that maximizes the complementary expertise of both disciplines in a multidisciplinary approach to head and neck conditions.

Clinical activities include care provided through inpatient services, Day Surgery, and Day Service programs, addressing oral, sinonasal, and cervicofacial conditions.

The Unit offers the complete spectrum of traumatic, dysmorphic, and oncologic cervicofacial-maxillofacial surgery, utilizing the most advanced therapeutic strategies, including: Reconstructive microsurgery, Robotic surgery with the Da Vinci Surgical System, Endoscopy with 3D visualization and navigation, Intraoperative neuromonitoring. Patients receive personalized treatment plans aimed at delivering the highest standard of care and optimizing clinical outcomes.

The surgical team operates across five weekly dedicated operating rooms, in addition to a room for local anesthesia procedures and facilities for emergency interventions. 

The activities cover all cervicofacial and maxillofacial pathologies, with a particular focus on:

Maxillofacial Traumatology

Maxillofacial traumatology focuses on the management of injuries to the maxillofacial region. Trauma occurs when external forces exceed the strength of the bones, leading to fractures.

Maxillary fractures are categorized as partial or total: Partial fractures affect the alveolar ridge, maxillary tuberosity, incisive bone, or palatal vault, Total fractures include Le Fort I, II, III fractures and intermaxillary disjunctions.

The Unit employs the most advanced equipment, including the Intraoperative Navigator, portable CT scanner, and 3D modeling technology, to ensure precise diagnostic and surgical outcomes.

Oncologic Surgery

Head and neck oncologic surgery addresses the surgical management of neoplastic conditions involving: Facial skin, ears, nose, and paranasal sinuses, Nasopharynx, oropharynx, hypopharynx, and larynx, Oral cavity and cervical trachea, Thyroid and parathyroid glands, Major salivary glands (parotid and submandibular) and minor salivary glands, Cervical lymph nodes.

Surgical interventions follow rigorous staging assessments and are planned collaboratively within a multidisciplinary oncology team, ensuring the most appropriate and tailored therapeutic strategy for each patient.

Procedures typically involve the excision of the primary tumor site. When applicable, the Unit employs minimally invasive techniques, including: Sentinel lymph node biopsy, CO2 laser surgery, Transoral Robotic Surgery (TORS) with the Da Vinci Surgical System.

Reconstructive and Microvascular Surgery

Reconstructive and microvascular surgery addresses extensive lesions in anatomically complex regions such as the head and neck. Advanced techniques are used to achieve anatomical and functional reconstruction of bones, blood vessels, muscles, nerves, and tendons. These include: Locoregional flaps, such as temporal and pectoral flaps, Microvascular flaps, such as radial, ALT (anterolateral thigh), latissimus dorsi, groin-SCIP (superficial circumflex iliac perforator), gracilis, fibula, scapular, and parascapular flaps.

Advanced Oral Surgery

Advanced oral surgery focuses on managing pathological and para-physiological conditions of the oral cavity and maxillary bones. Common procedures include: Complex extractions, Biopsies for diagnosing oral lesions, Frenulectomy of the lips or tongue, Preprosthetic surgery, Surgical management of odontogenic infections, Maxillary sinus surgery.

Surgery for Facial Dysmorphisms

Facial dysmorphism surgery involves the diagnosis and treatment of maxillomandibular malformations, which disrupt normal dentoskeletal relationships.

Treatment goals include: Restoring stomatognathic function, Enhancing aesthetic harmony.

OSAS

For sleep-related breathing disorders, including snoring and obstructive sleep apnea syndrome (OSAS), surgical interventions are performed on appropriately selected patients based on sleep endoscopy findings. These include: Tonsillectomy, Pharyngoplasty, Epiglottoplasty, Lingual base volume reduction using the Da Vinci Robotic System (TORS).

Sinonasal Surgery

Sinonasal surgery addresses conditions such as: Chronic rhinosinusitis and its complications (e.g., orbital or intracranial involvement), Nasal polyps, including antrochoanal polyps, Benign sinonasal conditions, such as inverted papilloma, juvenile angiofibroma, and unilateral or bilateral choanal atresia, Sinonasal malignancies. Skull base defects, including cerebrospinal fluid leaks, meningoceles, and meningoencephaloceles.

Additionally, sinonasal approaches enable the treatment of: Lacrimal duct disorders, such as chronic dacryocystitis or epiphora, via dacryocystorhinostomy, Orbital conditions, such as cavernous angiomas

Salivary Gland Surgery

Salivary gland surgery focuses on disorders affecting the: Major salivary glands, including the parotid, submandibular, and sublingual glands, Minor salivary glands.

These conditions may be benign (e.g., pleomorphic adenoma, Warthin’s tumor) or malignant (e.g., adenocarcinoma, mucoepidermoid carcinoma).

Surgical procedures prioritize the preservation of the facial nerve, using advanced neuromonitoring systems to ensure safety and precision.

Robotic Surgery (TORS)

Robotic surgery for head and neck disorders is primarily used to treat: Selected tumors of the upper aerodigestive tract, including the oropharynx, hypopharynx, and larynx; OSAS, through lingual base volume reduction.

Pediatric Maxillofacial Surgery

Pediatric maxillofacial surgery focuses on malformative and reconstructive head and neck procedures in children, including: Cleft lip and palate repair, Branchial cyst and fistula removal, Thyroglossal duct cyst and fistula excision.

Multidisciplinary Oncology Head and Neck Outpatient Clinic

Robotic surgery for head and neck disorders is primarily used to treat: Selected tumors of the upper aerodigestive tract, including the oropharynx, hypopharynx, and larynx; OSAS, through lingual base volume reduction.

 This clinic operates every Monday morning at the Giovanni Paolo II Polyclinic, ensuring patients receive diagnostic and therapeutic care aligned with international standards. Appointments can be scheduled through the CUP system, with a referral for “PCA88” or “PCA89.”
