Pain Management Therapy

Pain management therapy, also known as "antalgic therapy" or "pain medicine," focuses on the diagnosis and treatment of patients suffering primarily from chronic, benign, or oncologic pain.

The concept of “pain management and patient care” often necessitates a multidisciplinary approach, involving collaboration among various specialists to develop a diagnostic-therapeutic pathway. This approach integrates pharmacological, invasive, instrumental, psychological, and rehabilitative therapies tailored to the individual. Pain, in its initial stages, serves as a physiological response that signals injury or an ongoing alteration in the body, functioning as a critical defense mechanism. However, when pain persists beyond the healing of the original injury, it ceases to serve its purpose as a warning signal and instead becomes a pathological condition in its own right.

Many conditions can trigger chronic pain. If inadequately treated, this pain can lead to psychological and social challenges, including anxiety and depression, significantly deteriorating the patient’s quality of life. Such impacts often necessitate a biopsychosocial approach to therapy.
 

In Italy, Law No. 38 of March 15, 2010, guarantees every citizen the right to access pain management therapy and palliative care. This legislation established guidelines for the creation of integrated care networks, linking community-based healthcare with hospital-based services. Pain management therapy is classified as part of the Essential Levels of Care (LEA), meaning it is a core health service provided by the National Health System at no cost to the patient.

Casa Sollievo della Sofferenza is a key center within the regional pain management network, serving as a reference point for pain treatment. Outpatient consultations and first-line therapies are offered at the Giovanni Paolo II Polyclinic from Monday to Friday. Comprehensive therapeutic approaches are available at the hospital and include: Multimodal pharmacological therapies, Minimally invasive procedures (e.g., regenerative biological product injections, central and peripheral nerve blocks, percutaneous neuromodulation techniques like radiofrequency, and nerve decompression), Major surgical interventions (e.g., neuroplastic and spinal endoscopic procedures, neuromodulation techniques for spinal and peripheral nerve stimulation, implantation of electronic pumps for spinal pharmacological neuromodulation), Biological support therapies and complementary medicine, Outcome monitoring and evaluation.

The Pain Management Therapy Unit treats a wide range of chronic pain conditions, including:

• Oncologic pain, including vertebral and extra vertebral bone metastasis-related pain
• Neuropathic pain syndromes, such as trigeminal neuralgia, post-stroke central pain, postherpetic neuralgia, and pain or spasticity associated with multiple sclerosis and other degenerative neurological conditions
• Peripheral neuropathies, including diabetic neuropathy, meralgia paresthetica, Arnold’s neuralgia, pudendal neuralgia, and chemotherapy-induced neuropathies
• Complex regional pain syndromes resulting from trauma or nerve injury
• Headaches and craniofacial pain syndromes
• Cervical, thoracic, and lumbar radiculopathies due to herniated discs or discopathies
• Chronic spinal pain, including lower back pain, dorsal pain, neck pain, and sacroiliac joint pain
• Post-surgical pain syndromes, such as pain following hernia repair, thoracotomies, mastectomies, or spinal surgeries, as well as pain resulting from treatments like radiotherapy
• Chronic pelvic and perineal pain, including coccydynia
• Ischemic pain associated with chronic vascular diseases, phantom limb pain, and post-amputation pain
• Rheumatic diseases, including rheumatoid arthritis and other chronic conditions
• Osteoarthritis affecting major and minor joints, such as the hips, shoulders, knees, and ankles
• Musculoskeletal pain, including tendinopathies, muscle strains, post-traumatic injuries, and sports-related pain
• Fibromyalgia and generalized widespread pain
